Once completed, the application can be emailed to avondale@tcbinc.org, faxed to 513-322-6828, mailed to or dropped off in person at 3635 Reading Road, Cincinnati, OH 45229, Monday-Friday 9am-5pm (pre-application is not deemed received and will not be time/date stamped until the application fee is received by the office.
